This is an order form for my youth group's easter candy sale I designed in high school. It is simply a two sided, 5.5 x 8.5 sheet I put together in Adobe Illustrator. So here you see the front and the back. We made the candy early so I was able to take pictures and put together this form before, so we could better sell the candy.
